Pharmacy franchising looks different from most franchising. A few brands sell a full retail pharmacy franchise to a pharmacist who opens a new store or converts one. The largest names are programs for independent pharmacies: the owner keeps the business and gets the brand, buying terms through a drug wholesaler, store design and marketing, often at a low fee. Infusion pharmacies prepare drugs given by IV at home or in clinics, a specialist and more costly model.

Every pharmacy needs a licensed pharmacist in charge, and most of these brands sell only to pharmacists or to owners who employ one.

The typical brand here costs $422K at the low end of its estimate and $735K at the high end. The lowest starting cost is Pharmaconic’s, from $322K. Use each brand’s full range for budgeting: the low end is a small site in a cheaper market.

Where the money goes: share of a typical brand's high-end cost to open (5 brands with a full breakdown)

  • Equipment, furniture and signs25%
  • Building, rent and fit-out22%
  • Insurance, licenses and other15%
  • Inventory and supplies7%
  • Franchise fee2%

A new pharmacy spends most on the store fit-out, the dispensing equipment and software, opening inventory of drugs and working capital, because insurers and pharmacy benefit managers pay for prescriptions weeks after they are filled. The affiliation programs cost little, since the pharmacy already exists.

Fees and royalties

The typical franchise fee is $0. Most of these brands charge a flat, tiered or other royalty rather than one percentage, so check each brand’s page. Royalties come off sales before rent, wages or loan payments. The fees and royalties guide compares industries.

What locations sell

One brand here publishes a comparable yearly sales figure per location. Sales are before costs, so they say nothing on their own about profit.

Which pharmacy franchises are growing

Of the 3 brands with three years of location figures, 2 grew from 2023 to 2025 and 1 shrank, a net change of +7 locations between them. On the SBA Franchise Directory are 2 of the 6, so SBA-backed loans are open to them.

Before you buy

Prescription reimbursement, set by pharmacy benefit managers, decides an independent pharmacy’s margin. Ask what the brand does to improve those rates or its owners’ buying prices, and what share of a typical store’s profit comes from front-of-store sales, vaccinations and other services. Check the state board of pharmacy’s rules for opening or transferring a license.

Questions

Do you have to be a pharmacist to own a pharmacy franchise?

Not in every state, but the pharmacy must have a licensed pharmacist in charge, and many pharmacy brands sell only to pharmacists. Some states limit who can own a pharmacy. Check your state board of pharmacy's ownership rules before you apply.

What does a pharmacy affiliation program give an independent pharmacy?

A recognized name and signs, better buying terms through the program's wholesaler, help negotiating with insurers and pharmacy benefit managers, and marketing and store-design support. The owner keeps the business. Fees are usually low or tied to buying from the wholesaler, so compare the buying terms more than the fee.
